What Is septal perforation?

A septal perforation refers to a hole that penetrates both the mucosa and underlying cartilage or bone of the nasal septum, creating an abnormal passage between the two nasal cavities.


It most commonly occurs as a complication after septoplasty, but can also result from trauma, nasal cauterization for epistaxis, foreign bodies such as button batteries, frequent nose picking, or long-term use of steroid nasal sprays. Patients may experience nasal obstruction, recurrent nosebleeds, crusting, or a whistling sound during breathing—symptoms that often require surgical correction. Surgical repair of septal perforation is a highly complex procedure with a moderate success rate and potential complications.


The operation involves reparing the perforated mucosa and replacing the missing cartilage or bone with graft material.


It is time-consuming and technically demanding, with a risk of worsening the perforation if not performed properly. Therefore, it requires highly specialized expertise.
